mothers2mothers (m2m) is an African Primary Health Care organisation that employs women living with HIV as community health workers across 10 African nations. These “Mentor Mothers” deliver integrated health services at clinics and in communities to ensure everyone, everywhere, has access to the services they need to be healthy and stay in care. Over the next five years, this proven, peer-led model is being scaled up to double down on ending HIV, as well as to tackle new health challenges and reach more people – all with the aim of building a fairer, healthier future. The employment of local women as community health workers means that we are delivering a model that prioritises healthcare for families who need it most, delivered by women who know them best. Since its creation in 2001, m2m has created jobs for nearly 12,000 women living with HIV in sub-Saharan Africa and, together, they have reached over 15M people with critical health services and education. Job Title: Community Social WorkerClosing Date: 13/9/2024Job DescriptionThe Community Social workers will address the case management needs of the Bokamoso OVC beneficiaries. She/he will work in coordination with other partner agencies to address the broad needs of the Bokamoso Participants/beneficiaries through the referral and linkage to eligible services. She/he will act as a core coordinator who will perform and coordinate community outreaches, prevention, and risk reduction, post-trauma psychosocial support provision, health education, and a follow-up on beneficiaries that were referred to ensure successful linkages and completion.
Key Performance Areas:
Oversee the technical implementation of OVC Comprehensive Case Management
• Prepare the case-related reports including outcomes, successes, and challenges
• Consolidate care plans submitted by the Case Management Workers for beneficiaries with short, medium, and long-term goals and the steps to achieve those goals
• Regularly conduct supportive supervision and mentorship to the Case Management Workers on the technical approach
• In Liaison with the Linkage Officers, initiate and participate in case conferences for complex cases
• Prepare case-related reports, including outcomes, successes, and challenges, and routinely share them with the line manager to improve implementation
• Advocate for the project participants with other service providers to minimize the barriers to them receiving needed services
• Follow up with referrals to ensure that project participants receive and access necessary services in a timely manner
• Oversee the activities done by the different case management workers and administer programs, and allocate resources to community services
M & E and Quality Assurance and Quality Improvement
• Ensure completeness and data quality to optimize swift and accurate capturing
• Data driven and make programmatic decision based on the results demonstrated by the data
• Ensure that the data shared for capturing is of high quality, appropriate and timely submitted
Administration, Financial management and HR support activities
• Undertake performance appraisals for Case Management Workers
• Facilitate and evaluate individual CMW and identify training needs CMW and facilitate the on-site training
• Collaborate with HR officer to ensure that all disciplinary issues are managed accurately and timeously
• Conduct regular site supportive supervisory visits through routine travel across the community councils in the district
Representation, stakeholder engagement and relationship management
• Establish and maintain collaborative working relationships with community stakeholders at District level
• Participate in relevant community stakeholder forums and provide feedback on progress within the project
Qualifications and experience:
• Degree in Social Work
• Training in Child Protection, Project Planning & Management and Counselling will be an added advantage
• At least 3 years’ experience in working with OVC programming on large project and experience with USAID funded programs/projects necessary
• Experience with implementing OVC interventions, knowledge of HIV/AIDS service delivery among positive caregivers and children
• Practice in an ethical manner and embrace diversity
