mothers2mothers (m2m) is an African Primary Health Care organisation that employs women living with HIV as community health workers across 10 African nations. These “Mentor Mothers” deliver integrated health services at clinics and in communities to ensure everyone, everywhere, has access to the services they need to be healthy and stay in care. Over the next five years, this proven, peer-led model is being scaled up to double down on ending HIV, as well as to tackle new health challenges and reach more people – all with the aim of building a fairer, healthier future. Job Title: Social Worker (Facility Based)Closing Date: 13/9/2024Job DescriptionThe Social Worker provides effective and efficient coordination of case management processes for children living with HIV (CLHIV) in health facilities. Facilitate the coordination of bi-directional linkages for the AGYW that are being referred to the health facility to access clinical services. S/he will contribute to the monitoring and reporting through the administering the case management tools in order to document CLHIV case referrals.
Key Performance Areas:
Lead the bi-directional referral for OVC and AGYW to access the clinical services
• Coordinate the successful linkages of the beneficiaries that are layered to the facility from the community to access the Clinical services
• Follow up with the community Case Management Workers the beneficiaries that are being referred to the community for access of the OVC and DREAMS services at community level
• Routinely during the health education provide information on the Bokamoso services that are being offered both at facility and community level
• In collaboration with the facility staff identify the unsuppressed CLHIV, HEI for further support at community level
• Weekly basis track the referrals to ensure that the linkage was successful and report back to the referring team
• Provide basic information to health facility clients on Bokamoso programs offered by consortium partners, so that they may be enrolled with their consent
• Acts as entry point and Bokamoso contact person in health facilities and community councils assigned to
• Record and monitor Viral Load and MMD for children under the age of 18, to ensure that appropriate community-based support can be provided to those experiencing treatment challenges
• Track referrals from health facilities to community services and vice versa and ensure that referred clients get to the expected service points and make follow ups
• Participate in case conferencing of clients
• Understand and provide proper guidance on continuum of prevention, care and treatment of HIV/AIDS
• Mobilize health campaigns and dialogues on HTS for vulnerable children and their households
• Provide weekly, monthly and quarterly reports on the services provided
• Report referral processes and challenges to both the DHMT and Bokamoso management
• Perform any other duty assigned
M & E and Quality Assurance and Quality Improvement
• Ensure completeness and data quality to optimize swift and accurate capturing
• Data driven and make programmatic decision based on the results demonstrated by the data
• Ensure that the data shared for capturing is of high quality, appropriate and timely submitted
Representation, stakeholder engagement and relationship management
• Establish and maintain collaborative working relationships with other stakeholders at District level
• Participate in relevant facility stakeholder forums and provide feedback on progress within the project
• Represent Bokamoso at facility level
Qualifications and experience:
• Degree in Social work
• Training in Child Protection, Project Planning & Management and Counselling will be an added advantage
• At least 3 years’ experience in working with OVC programming on large project and experience with USAID funded programs/projects necessary.
• Experience with implementing OVC interventions, knowledge of HIV/AIDS service delivery among positive caregivers and children
• Practice in an ethical manner and embrace diversity
