Situated in the Leribe district; Lesotho, Phelisanang Bophelong HIV/AIDS Network, herein referred to as “PB”, is a Non-Governmental Organization (NGO) and a Community-Based Organization (CBO) of People Living with HIV and AIDS (PLWHA) established in 2004. PB’s core mandate is to improve quality of life for PLWHA, Most at risk populations, Key and other vulnerable populations through universal access to Prevention, Treatment, Care, and Support services.
The organization reaches beneficiaries with services ranging from Prevention of TB/HIV and AIDS to impact mitigation through Prevention, Support, and Protection services, Sexual and Reproductive Health and Rights (SRHR), Gender Based Violence (GBV) responses, Livelihoods and Advocacy interventions at the community level. PB invites suitably qualified applicants for the following positions supported under the USAID Sustaining Epidemic Control through Unified Prevention (SECURE) Activity. PB’s scope of work under SECURE is to improve coverage of non-discriminatory, client centred, integrated services across the continuum of care for key populations. The positions designed to support the HIV package of services for Female Sex Workers (FSW) in Maseru district.

The purpose of this position is to assist targeted populations, particularly those Living with HIV and on PrEP, to identify and overcome barriers that interfere with achieving health and related personal goals for ensuring that clients are effectively navigated to access both clinical and non – clinical health care services. Support adherence and retain in care for clients within the continuum of HIV prevention, care and support services. In collaboration with other peer staff:
Use the peer navigation tools to conduct one-on-one tailored post-test counseling and support to clients to identify their unique needs related to areas such as health well-being, self-care, emotional, psychological and spiritual wellness, socio-economic needs, disclosure, harm-reduction etc.
Support clients to develop self-management skills based on clients’-identified needs;
Refer and link clients to access and enrol in clinical, diagnostic, psychosocial, and other support services, including undertaking physical accompaniments to referred services;
Liaise with health and other social service providers as needed to ensure that clients linked to services are receiving the services as needed;
Support clients to adhere to their treatment regimens.
Carry out periodic and structured follow-ups for clients linked to further care, using various methods such as phone calls, home visits, emails etc., and document results for the follow-ups for each client using program tools.
Complete all the data collection tools and all relevant registers to report data on services provided to clients.
Maintain confidential and accurate clients’ files for peer navigation services.
Prepare and submit daily, weekly and monthly reports on peer navigation and patient tracking and follow up actions.
Basic requirements, Qualifications, skills and competencies
Must have completed COSC / LGCSE or Equivalent
Must have a minimum of one (1) year of work experience in supporting interventions for the targeted population
Previous experience as peer navigator a requirement
Good interpersonal and communication skills
Ability to work flexible hours including weekends
Ability to travel as and when required, and to work in challenging environment
Basic computer skills
Be a member of the targeted populations
