Job Title: Peer Navigator

Location: Maseru City Council and Qiloane Council

Experience: Minimum of 1 year experience in a related field.

Employment Type: Fixed-term contract

Reports to: Programmes Director

Job Purpose

The Peer Navigator will be responsible for assisting the key populations (KPs) reached within the program, particularly those living with HIV, to identify and overcome barriers that interfere with achieving health-related personal goals; for ensuring that clients are effectively navigated to access both clinical and non-clinical health care services; and, for supporting adherence and retain in care for clients within the continuum of HIV prevention, care and support services. In collaboration with the other peer staff at the program sites, and community and facility-based service providers within the network, the Peer Navigators will support effective referrals, peer navigation, linkages to clinical and non-clinical services, and retention in care for 20 to 40 program beneficiaries that include men who have sex with men (MSM) and transgender persons (TGs).

Duties and Responsibilities

The duties and responsibilities of the Peer Navigator will be:

 Use the peer navigation tools to conduct one-on-one tailored post-test counseling and support to MSM and TGs reached to identify their unique needs related to areas such as health well-being, self-care, emotional, psychological and spiritual wellness, socio-economic needs, disclosure, harm-reduction etc.

 Help participants to develop self-management skills based on clients’-identified needs;

 Refer and link KP clients to access and enroll in clinical, diagnostic, psychosocial, and other support services, including undertaking physical accompaniments to referred services;

 Working under the guidance of the supervisor, liaise with health and other social service providers as needed to ensure that KPs linked to services are receiving the services as needed;

 Support the KP clients to adhere to their treatment regimens;

 Carry out periodic and structured follow-ups for MSM and TGs linked to further care, using various methods such as phone calls, home visits, emails etc., and document results for the follow-ups for each client using program tools;

 Working under the guidance of the supervisor, organize and facilitate psychosocial groups and/or ongoing interactions with clients enrolled in care to ensure provision of continuous emotional and psychosocial support based on clients’ needs;

 Complete all the data collection tools and all relevant registers to report data on services provided to clients;

 Maintain confidential and accurate clients’ files for peer navigation services;

 Prepare and submit daily, weekly and monthly reports on peer navigation and patient tracking and follows; and,

 Undertake any other work assigned by the supervisor.

Requirements and Qualifications

Basic Requirements

 Must be willing to relocate to place of work (Qiloane or Maseru City Council)

 Good interpersonal and communication skills.

 Must have non-judgmental, trustworthy and empathetic personality.

 Must be an open member of the LGBTI community.

 Should be comfortable discussing issues of HIV/AIDS, sexuality, reproductive health as well as sexual orientation and identities.

 Should have the ability to handle a large workload in a professional, timely manner.

 Must have basic computer skills including use of Microsoft applications (Ms. Word, Excel, Outlook).

 Ability and willingness to work flexible hours and days of the week, including weekends.

 Ability to travel as and when required, and to work in challenging environments.

Education and Experience:

 Must have completed a senior high school certificate (i.e. COSC, LGCSE or equivalent)

 Must have a minimum of one (1) year of work experience in HIV prevention and care programs or related work. Prior training as a peer navigator or a peer educator for a KP HIV prevention, care and support program will be an added advantage.
